Youth Outpatient Services
At Momentum for Mental Health we recognize the challenges youth and young adults with mental illness face. Our TAY outpatient services offer a full service partnership (FSP) model, that is focused on helping young people and their families cope with mental illness and the trials it often brings.
Transitional Age Youth (TAY)
Transitional-Age Youth Services provide comprehensive outpatient mental health services, group therapy and skill development for young adults with a mental illness diagnosis (aged 14 to 24). Momentum’s TAY team is comprised of case managers, clinicians and psychiatrists who work together with this unique population of youth and young adults to stabilize any mental health concerns, provide personalized treatment plans for each individual, and foster a positive outlook on adulthood.
With these concerns in mind, we have created a supportive, intensive program that addresses age-specific needs from academic functioning all the way to Independent Living Programs like building a resume, managing a bank account and housing stabilization.
