Adult Acute Care
Adult acute care features our full service partnership programs.
Full Service Partnership (FSP) programs are designed for adults ages 26-59 who have been diagnosed with a severe mental illness and would benefit from an intensive service program. Full Service Partnerships provide a “whatever it takes” approach to support individuals on their path to recovery and wellness.
FSP uses methods and interventions that are person-centered and strengths-based. Peer, social and family networks play an important role in this recovery process. All services are voluntary and driven by individual choice. Through this collaborative effort, successful outcomes are achieved, and individual and family strengths are celebrated. Community resources are linked to support individual goals developed.
Adult FSP programs assist with housing, employment and education in addition to providing mental health services and integrated treatment for individuals such as intensive therapy, case management, rehabilitation, and medication support.
All referrals are made by the Santa Clara County Mental Health Department (800-704-0900). Adults may be Medi-Cal eligible or uninsured, and must meet medical necessity for treatment.
