TRANSITIONAL-AGE YOUTH (TAY)


The Transitional-Age Youth Services Program is an intensive community-based mental health program designed to stabilize crisis situations and resolve behaviors that interfere with the youth's ability to function at home, school, foster care, and/or in a group home.

The goal is for the youth/young adult to achieve the level of life management that helps them continue to live at home or the most home-like setting possible, without the need to enter residential treatment or an acute psychiatric facility, as well as to achieving their full personal potential. Services will be provided by various methods: Individual/family therapy, groups, case management, crisis intervention, in-home visits, and other supportive methods emphasizing youths to participate in community activities. Youths are also encouraged to utilize the program’s drop-in center (Club TAY) for activities, outings, and groups. No appointments are necessary to participate in Club TAY activities and some activities/outings may take place during off-hours. Services are sensitive to the needs of youth and families with diverse cultural backgrounds and partner with referrals from juvenile probation, county mental health, and department of social services.

Admissions Criteria:
14-24 years-old; ambulatory, mentally disordered classification - primary diagnosis on Axis I in the DSM-IV (Diagnostic & Statistical Manual of Mental Disorders); Santa Clara County resident; Medi-Cal (or Medi-Cal sponsored programs) beneficiary.


Exclusionary Criteria:
Private Insurance & Uninsured individuals (All referred individuals must be eligible and active Medi-Cal beneficiaries)

Referral Source:

Anyone may make a referral by calling the program directly. Individuals who are referred will meet with a Clinician, to assess for appropriateness to the program. Representatives from Group homes, residential programs, or other youth/young adult programs may refer individuals to programs or mental health/Rehabilitation services as well.

Fees:
Paid for by Medi-Cal/Short-Doyle or Healthy Families Insurance; individuals who do not have Medi-Cal or Healthy Families Insurance are admitted based on availability.
