Who is eligible for services?
Any individual between the ages of 14-24 years that have Medi-Cal or Healthy Families Insurance coverage, with at
least one Axis-I diagnosis is eligible for services. Clients will be transitioned into an Adult program, if necessary,
before their 21st birthday.
How can I refer an individual to your program?
Referrals can be made to our program by calling our line directly: (408) 254-6848 between office hours. Providing our
program with the individual’s first and last name, social security number, and date of birth with the referral call is
recommended, to ensure accelerated process. Referral calls do not guarantee admission to the program. Admissions will be
based on results of an initial Clinical Assessment, completed by the Program Clinician.
Can an adolescent under the age of 18 years refer their self to the program?
An individual under the age of 18 may call our program directly to refer their self. However, if the individual has a
guardian, the guardian (parent, Social Worker, Foster parent, etc.) will be required to sign a consent form for
treatment upon individual’s admission to the program.

What are your office hours?
General
Office hours for the program is Monday-Friday, 9:00am to 7:00pm. Fax hours
is Monday-Friday, 9:00am to 5:00pm at (408) 254-6856. If you are calling
during off hours or weekends, all program numbers have voicemail available
for you to leave messages.
