OUTPATIENT
Momentum offers many outpatient services, including case management, counseling,
benefits application, living skills and individually tailored services essential to
the achievement and maintenance of a stable and satisfying life in the community for
adolescents, adults and older adults. Services are available on a fee-for-service
basis and provided throughout the South San Francisco Bay area. These programs may
supplement services from another provider. Some insurance companies will cover the
cost of these services.
Case Management
Momentum service teams provide case management, rehabilitation and crisis intervention
for persons with persistent and severe mental illness. Services are provided throughout
Santa Clara County and include helping individuals access needed resources and other services.
Mental Health Services
Momentum professionals provide one-on-one and group therapy and/or assistance in
learning life management and coping skills to achieve greater self-reliance.
Through a contractual relationship with the Santa Clara County Mental Health Department,
Momentum offers services to eligible individuals. For information about the above services,
