DAY REHABILITATION
Momentum offers Day Rehabilitation Services in three sites throughout Santa Clara County.
Clients visit "Donna's Place", "Crossroads", and "Discovery Center" to receive help in achieving improved life management skills and greater independence. Services are provided in structured groups, drop-in centers and other self-help opportunities with a focus on client empowerment and recovery. Fees for services are accepted from clients and families.
Through a contractual relationship with Santa Clara County Mental Health Department, Momentum
provides services throughout Silicon Valley for eligible individuals. For information about the
