Who is eligible for services?
Any person who is diagnosed with a serious mental illness who has a diagnosis that meets medical necessity
requirements. They must be at least 18 years of age and ambulatory.
How can I refer an individual to your program?
Anyone can make a referral to our Residential Central Intake by calling 408.295.4723. There is a 'Pre-admission'
form the needs to be completed. It is a three page form that has all the requirements for admission i.e., current
diagnosis (Axis I, II, II), current medications, any history of self harm behaviors etc. This can be faxed to our
Residential Central Intake 408.295.4231. The Residential Central Intake will triage all client who meet the
criteria to one our three Crisis Programs.
How long can an individual receive services?
The programs are licensed by the California Department of Social Services and certified by the State Department of
Mental Health. Every client's length of stay is individually assessed. Average length of stay is approximately 2-3 weeks, with the maximum length of stay being 90 days.
What are your office hours?
The general office hours for our Residential Central Intake are Monday - Friday 9:00 to 5:00.
