COMPREHENSIVE SERVICES
Momentum provides unique services as part of a diverse system of care.
To provide a diverse and coordinated system of care, Momentum offers 12 distinct lines of service in 13 sites throughout Santa Clara County. These services include rehabilitation programs, seven multi-disciplinary service teams, two vocational service teams, long and short-term residential facilities and supported housing services. Peer counseling, self-help services and a variety of volunteer opportunities are also offered to assist clients in skill building.
